Insurance Claims

If you are on the UNC Student Health Insurance and need to file a claim for reimbursement, follow these instructions:

• Save all itemized bills to satisfy your deductible.
• An itemized bill from a Doctor or Hospital must include:
  • Name and address of provider
  • Tax Identification number of the health care provider
  • The name of the patient receiving services or supplies
  • The date services or supplies were provided
  • Each charge, service, and supply
  • A description of the services or supplies
  • Diagnosis or illness
