UNC Faculty & Staff Vaccination Policy
Employees should complete the online vaccine affidavit form by August 15, 2021. Faculty and staff may also indicate they request an exemption by using this form. Please use your self-service login and password to access the form.
Effective Sunday, August 15, 2021, the University of Northern Colorado requires employees to be vaccinated against COVID-19. Medical and non-medical exemptions are available to employees who are unable to receive a vaccination.
Employees must complete and submit an affidavit confirming they have received a full course of vaccinations against COVID-19 no later than Sunday, August 15, 2021 or receive a medical or non-medical exemption. UNC’s secure portal for submitting vaccination affidavits and exemption documentation is now open. New employees hired after this date and for the duration of the policy must prove they have received at least a first dose before they may begin work.
Employees may be vaccinated at a vaccination clinic or by a healthcare provider of their choice and receive any of the vaccines currently available.
On the policy’s effective date of August 15, the university will begin contacting employees who have not submitted an affidavit or been granted an exemption to help them to comply with the policy. In the absence of an approved exemption, those who fail to comply with or complete the COVID-19 vaccination requirement by Monday, August 23, 2021 will be subject to penalties, as defined below.
If none of the vaccines available in the United States are approved by the US Food and Drug Administration or, if in its sole discretion, UNC determines that there is insufficient COVID-19 vaccine supply to meet the demand, the University may either delay implementation of this requirement or amend the same.How to Submit Your Records
UNC’s secure portal for submitting records and exemption documentation is now open. Employees should complete their affidavit or exemption request through UNC’s secure portal no later than August 15, 2021. You will be prompted to answer a question to validate you have received the vaccine or are seeking an exemption.
Exemption From Policy
Employees have the right to request an exemption from this vaccination requirement
for a medical or non-medical (religious or personal belief) reason. This policy adopts
the terms and definitions used by the State of Colorado for other vaccinations. To
request an exemption, employees should complete the online form and answer the appropriate questions.
If granted an exemption, employees may be required* to:
- Participate in regular COVID-19 testing**
- Monitor UNC email daily for important health notifications
- Isolate for 10 days if tested positive for COVID-19
- Quarantine for 14 days if the employee is determined to be a close contact of a positive case by the UNC, Weld County Department of Public Health and Environment, or another state or local public health authority.
*These requirements are subject to change based upon available and emerging epidemiological evidence and the overall burden of disease related to COVID-19 transmission on our campus and in our region. Employee compliance with these requirements is subject to verification.
**The specific process for weekly testing and cost to the employee will depend on available testing options at the time.
Failure to Comply
Employees who have not confirmed they have received at least one vaccination dose or been granted an exemption prior to Monday, August 23, 2021 will be subject to disciplinary action up to and including termination in accordance with existing disciplinary processes for the applicable employee classification.
Appeals will be considered on a case-by-case basis utilizing an interactive process with the employee.
