CLINICAL ROTATION EVALUATION INFORMATION:

At the end of each 5 week clinical rotation, the ATS, the ACI, and the Clinical Rotation are evaluated using the forms below.  The ATS and ACI should schedule a meeting sometime during the final week of each rotation to compare and discuss the ATS evaluation and ATS self-evaluation forms as well as the rotation in general.  Both forms should be signed and dated by both parties, and the ATS should collect both forms and turn them in to the Clinical Coordinator by 9am on the following Monday.  

The Clinical Site & ACI Evaluation form should be completed by the student anonymously and turned into the Clinical Coordinator separately of the ATS evaluation forms.  The Clinical Site & ACI Self-Evaluation form should be filled out by each ACI and turned into the Clinical Coordinator separately of the ATS evaluation forms.

STUDENT SEMESTER COMPETENCIES AND SEMESTER EVALUATIONS

The clinical athletic training student purchases a semester competency binder at the beginning of their clinical program. This competency binder outlines the competencies required each semester, as well as their competency check off sheets. It also contains the policies and procedures for the clinical athletic training program. We continually go over these policies and procedures each semester and update them equally as often, you should bring your handbook with you every day.

At the end of each semester all ACI's participate in a comprehensive review of each athletic training student and the student is either:

    1. Encouraged to continue in the program.
    2. Encouraged to continue in the program with major improvement needed.
    3. Encouraged to continue but must complete previous semester goals.
    4. Placed on probation that can include temporary suspension from clinical experiences.
    5. Dropped from the clinical program.

Goals are set each semester based on the approved athletic training competencies and a student’s clinical progression. These evaluations are discussed with the athletic training students and they receive a copy.

Athletic training students placed on university probation will automatically be placed on probation in the athletic training clinical program. If an athletic training student falls below a 2.8 cumulative GPA, they will be placed on probation in the athletic training education program. Probation includes a reduction of their clinical work load and team assignments. Their GPA must be brought back up the following semester or they will be dismissed from the program.

All students in required athletic training courses that receive a grade of "D" or "F" must retake the course until a grade of "C" or better is obtained. An athletic training student receiving grades of "D" or "F" in two clinical athletic training classes will be dismissed from the program.
