Colorado
law and State Health Department regulations require all
students, undergraduate and graduate, enrolled in colleges and
universities to submit proof of immunization.* Students must
submit the completed Certificate of Immunization or Statement
of Exemption form to the Student Health Center before
registration. If you are planning to register by mail, include
your immunization, Student Information, and Registration
(Course Selection by Mail) forms in the packet for the
Registration Center.
If
your proof of immunity is incomplete, contact the Student
Health Center to sign an agreement to submit complete
information within 14 days. You will not be allowed to
register for the next term unless the agreement is satisfied,
per Colorado State Statute
A

student who is enrolled for one or more classes at a college

but students who are taking only correspondence or off-campus
