Exiting the University
All employees who leave the University should participate in the check-out process to ensure you receive all relevant information based on your position and situation.
Exit Interview: Whenever possible, all full- & part-time regular (non-temporary) employees should schedule an exit interview with Human Resource Services. A coordinator will review any benefits or other items with you at that time.
Exit Interview Survey: Human Resource Services has created an online exit survey to capture information regarding your work experience with the University. All employees who leave the University are encouraged to complete the survey. To complete the survey, go to
For Faculty/Professional Administrative Staff Separations, supervisors need to ensure a contract data form is completed and submitted to Payroll to communicated when an employee is leaving and avoid any over/under payments to the employee. Additionally, complete the Termination /Retirement form and submit to Human Resources so we can determine final payouts for the employee.
For Classified Staff, submit the Termination/Retirement form to HRS.
Supervisors should complete the appropriate evaluation form for exiting employees and submit to HRS within a month of separation.
