Survey Gizmo Use

GSA has an enterprise subscription with the survey website Survey Gizmo.com. Through this subscription, GSA can issue out a certain number of individual accounts to UNC grad students for research use.

 

For this trial program, we are assigning accounts for 2 weeks. GSA strongly suggests you sign up for a free account at www.surveygizmo.com and spend sometime practicing how to create the survey and so forth. This way you will be able to quickly build the survey once you have access to the account.

 

GSA is looking into having people create surveys in the free account and then transfer it to their GSA account, but currently that is not an option.

 

The use of Surveygizmo is for research only. If you need to do a survey as part of a class requirement, please have the instructor contact GSA (gsa@unco.edu) and an arrangement will be made so that a class may have use of a single Surveygizmo account. This will ensure that as many people get to use the survey system and we do not have a single class "clogging" up the accounts.

 

A GSA account will allow grad students to use Surveygizmo's website to create a variety of surveys that have different abilities.  These Include:

  • Unlimited number of surveys
  • Up to 50,000 respondents per survey
  • Putting images and other media files in your survey
  • Randomize questions, responses, or page order
  • Page skip logic (Create survey branches the old fashion way,jumping between pages.)
  • More advance skip logic, based on question and/or question piping; also disqualification logic, specialized scoring.
  • Likert scale, rating scale, drop down menus, check boxes, radio boxes, text boxes and pretty much any other response format you need.
  • Save and continue for participants; participants can save their entries and return later to complete the survey.
  • Secure surveys with https://
  • Geo tracking via IP
  • Email invites
  • Export data to CSV, Excel, or SPSS!

Rules and Policies Concerning GSA Surveygizmo usage.

  1. Any UNC graduate student is in good standing with the University and is enrolled in at least one (1) credit hour may apply to use GSA Surveygizmo account.
  2. Any student who wishes to use the GSA Surveygizmo account must submit a complete application (available below), an IRB approval form, and an outline of their study (grad student may use the study description from their IRB narrative).
    • The GSA President will only review surveys for the purpose of review to ensure compliance with IRB guidelines. All survey items, data, and other materials created by a student with the GSA Surveygizmo account will be considered privileged information and GSA will not allow any other students to access it, with exception to reasons stated in paragraph three (3).
  3. The GSA President reserves the right to review any survey and stop an applicant's usage at anytime for any reason. Any data gathered will be turned over to the applicant, with the exception of students are believed to be violating UNC IRB procedures, in which case the data will be surrendered to the IRB committee.
  4. Upon completion, all data will be taken (downloaded) by the student and any surveys on the account will be deleted, either by the student or GSA President.

 

UNC Graduate Students may apply for an account by filling out the following application. This is an interactive Adobe PDF, so you fill in the required information and then print it out. GSA would strongly prefer appications that were printed out. Near the top of the Adobe Reader, there should be a check box that will highlight the required fields, which you can then put your mouse cursor over and fill in. Be sure to sign it once you have printed it out.

Turn in the application to the GSA President. After a review, the GSA president will email you and let you know the dates of your use and your account username and password; or I will tell you the denial.

 

Please note, for ethical considerations and to make sure people who are given an account have a good and faithful intention of using the account, GSA is requiring applicants to submit a copy their Institutional Review Board (IRB) approval letters. Any research concerning human subjects has to be approved by UNC's IRB. You can read about the IRB and obtain the appropriate forms from the SPARC website.
