By signing this form, you agree that uniforms supplied to employees are for use during working hours only and is considered University property. Uniforms must be returned in good condition prior to the release of the final paycheck. Failure to return such property upon departure from employment with the University may result in the garnishing of the employees pay to compensate for the cost of the uniform.
