They have been saved as Acrobat PDF and Microsoft Word files and may be saved and filled out on your computer and then printed for mailing or faxing. You may also fill them out by hand. To get the free acrobat reader, visit Adobe's site.

Please contact the program director regarding any questions, suggestions, or problems you may have. It is our intention to make this experience for you a dynamic one so that you may experience the benefits of mentoring a beginning professional in our field.

We need to document to the Commission on Accreditation for Dietetics Education (CADE) that the intern has completed the required number of hours and rotations during the internship. There are master copies of the evaluation forms used in the intern's manual as well.

As the preceptor, we require that you see, review, and sign off on all the documentation pertaining to the intern practicum experiences.

The documentation provides a natural “trigger” for discussion at regular intervals regarding intern progress. Your ongoing evaluations will allow the intern feedback so that there are no surprises regarding actual capabilities, academic and professional expectations, evaluations, and the results of the evaluations.

Part V: ADA Competencies and Assignments - The rotation evaluations reference these competencies set by the American Dietetic Association.

Part VI: Intern Professional Conduct Guidelines - The intern's failure to follow these guidelines could result in termination from the program.
