The following links are to the official UNC, School of Teacher Education evaluation tools used to manage our teacher preparation programs. Please read carefully to ensure that the correct survey is chosen. If you have any questions, contact the STE office at: 970-351-2908. THANK YOU.

Practicum Candidates-These surveys evaluate UNC practicum candidates in the undergraduate and graduate level Elementary and Early Childhood programs. 'Practicum' indicates the candidates are in their host school part-time.

Student Teaching Candidates-These surveys evaluate UNC student teaching candidates. 'Student Teaching' indicates the candidates are in their host school full time.

UNDERGRADUATE PROGRAMS:

  • Elementary, on-campus only, mid-term, completed by the Cooperating Teacher
  • Elementary & Early Childhood final evaluation, completed by the Cooperating Teacher and UNC Supervisor
  • Center For Urban Education Elementary (Denver's Lowry Campus) final evaluation, completed by the Cooperating Teacher and UNC Supervisor
  • Secondary & K-12 ART & MUSIC final evaluation, completed by the Cooperating Teacher and UNC Supervisor
  • Teacher Education Program Exit Survey, completed by teacher candidates ONLY in ALL program areas

GRADUATE PROGRAMS, Master's and Post Baccalaureate:

  • MAT: Elementary Mid-Term, completed by the Cooperating Teacher
  • MAT Elementary final evaluation, completed by the Cooperating Teacher and UNC Supervisor
  • Post Bac Secondary & K-12 Art & MUSIC final evaluation, completed by the Cooperating Teacher and UNC Supervisor
  • Teacher Education Program Exit Survey, completed by teacher candidates ONLY in ALL program areas
