Refer a Youth

Referral Information

UNC Campus Connections accepts referrals for youth, ages 11-18, who live in Weld County. Many of our referrals come from our community partners, including from the Juvenile Court Magistrate, Weld County Office of the District Attorney, Weld County Probation, and School Resource Officers and School Counselors.

We also accept referrals directly from family members. Many youth and their families choose to return to Campus Connections and send siblings even when this is not required.

Referrals are accepted at any time. We make every effort to enroll all referred youth as space permits. We strive to have youth begin during the first week of each session (fall, spring), however, referred youth can start throughout the 12-weeks, as space permits.

Referral Process

After receiving the referral form, a member of the Campus Connections team will call and schedule an intake appointment with the youth and their parent(s)/guardian(s). After this initial appointment, the youth will be registered for Campus Connections.
